Waiver & Community-Based Services
We provide non-medical support through Pennsylvania’s home and community-based programs, helping individuals remain safely in their homes while receiving the care they need.
Our team works with families to navigate available services and provide care based on eligibility and authorization.
Support with everyday tasks such as bathing, dressing, mobility, and daily routines, helping individuals remain safe and independent at home.
Services for individuals who do not qualify for Medicaid but still need support at home. This program provides personal care and assistance to help maintain independence.
Home and community-based services for individuals with physical disabilities, supporting daily living and independence with authorized care.
Short-term support that allows family caregivers time to rest while ensuring their loved one continues to receive safe, reliable care.
Long-term services and supports for eligible individuals through Pennsylvania’s Community HealthChoices program. Services are subject to eligibility, authorization, and managed care organization (MCO) credentialing.
