What to expect during the assessment
Your initial consultation is a personalized, in-home visit designed to understand your needs and create a care plan that supports your comfort, independence, and lifestyle.
During this visit, your dedicated Care Coordinator will:
Get to know you and your family, building a foundation of trust and understanding
Provide an overview of our state-licensed and fully insured agency, Intercede Home Healthcare
Discuss your daily routines, personal preferences, and household dynamics
Review any mobility needs, support requirements, and general health considerations
Review important documents such as Advance Directives or Power of Attorney (if available)
Identify potential safety considerations within the home to promote a secure environment
Answer your questions and guide you through your care options
Our goal is to ensure you feel informed, supported, and confident in moving forward with care.
What do I need to begin services?
If you decide to move forward, we will guide you through a simple onboarding process, including:
Service Agreement
Consent for Services
HIPAA Acknowledgment
If available, we may also review:
A current list of medications
Advance Directives or DNR documentation
Medical Power of Attorney
If these documents are not readily available, our team can assist you in gathering what’s needed.

Consultation Information
We offer an initial in-home consultation to understand your needs, answer questions, and develop a personalized care plan tailored to your situation.
For private-pay clients, this consultation is a professional service designed to ensure we can provide the right level of care and a caregiver match from the start.
A consultation fee may apply and will be discussed prior to scheduling. Any applicable fee is credited toward your first invoice if services are initiated.
For individuals receiving state-funded services (including Medicaid or waiver programs), assessments and service coordination are completed in accordance with program guidelines.
